The Resource Coordinator manages the resource program, oversees admins, and focuses on database integrity, relationships with resource providers as well as primary care physician offices.

Care Navigator

The Care Navigator position is responsible for the enrollment and activation processes, services both inbound and outbound communication and assists in determining what support is needed.

Registered Nurse

The Registered Nurse role will provide clinical assessments, appropriate proactive solutions to help avoid potential hospitalizations, and clinical education and information on the stages of Alzheimer’s disease and Dementia.
