The goal of VirtuALZ is to diffuse difficult day-to-day challenges. When members are having a tough day, caregivers can call in and speak to the VirtuALZ Care Team. The Team will develop a relationship with the Caregivers and their loved one thereby allowing them to offer personal, immediate guidance and support. The Care Navigator will be available to assess any situations that arise and connect the Caregiver to a nurse or social worker. The consistent, ongoing support and connection with the Caregiver and their loved one can reduce unnecessary emergency room or urgent care visits.
Having a situation or condition triaged in the safety and privacy of one’s home is the first step in reducing the medical costs associated with caring for a loved one with Alzheimer’s Disease or Dementia.
